Joseph "Jay" Kujawa IV

Predeceased by his parents, his brother Danny, and many other loved ones. Survived by his brother Doug, who took care of him in his finals days. His sister Lynne, his nieces and nephews as well as many other family members and friends who loved him. Also survived by his daughter. Jay will be missed by all who loved him. Jay was a diehard Notre Dame fan as well as the NY Giants.
